Please fill up pages 22-26 of this Form, for your initial investment. Page 27 contains a grid which mentions the relevant documentation to be attached alongwith the Form.

In case you are submitting both the applications (Lumpsum + SIP) concurrently, you need not attach two separate cheques, as long as you are using the same bank account for both the transactions.

You may deposit this Form either at your nearest CAMS Investor Service Centre or send it to any of our branches.
